Directions: Once in the parking lot of the larger building, go in the left entrance. Once inside the building you may take the stairs (to the right) or the elevator (to the left) to the second floor. Once on the second floor, go into Suite 215 “Horizon Health” and wait in the waiting area for your appointment.
